Blue and Aqua heart with medical herald logo, which can be kept as is or changed to your add your own company logo by easily changing image on template. Area for full name, job title, hospital or company name and ID number (if id number is not required, you can erase characters from text box to remove or add instead any other information you like) section for company /hospital name.
