This is the Magician card, part of the Major Arcana of a tarot deck. It's a hand drawn image that's been digitally painted. It depicts a mysterious with winged hand and foot holding onto a bag whose contents are unknown He's standing on a plinth enclosing the four elements and surrounded by a caduceus wrapped around a glowing pillar on either side.
