I have been teaching on-line since 2009, and I use this to organise my comments about the students. I use this during a lesson to make notes, so when I write my evaluation at the end of the lesson I can use this information to make a informed report to the parents. I think that you can use my form and be a more effective on-line teacher.

Love this tracker! The paper is a nice weight, the boxes are big enough to write in easily, and it is easy to add dates for each month. It is also very easy to switch up your categories each month as your needs change, and there are plenty of lines. This was the second one I ordered and it is just as nice as the first one was.
